A 42YEARS MALE WITH YELLOWISH DISCOLORATION OF SCLERA AND URINE

CHEIF COMPLIANT:
A 42 year old male who is farmer by occupation came with the chief complaint of yellowish discoloration of sclera and urine since 1week
HISTORY OF PRESENT ILLNESS:
patient was apparently asymptomatic 5 years back,then had similar complaint in the past and was ob for alcoholic abstinence.patient had a complaint of vomitings and loose stools 6to 7times a day 7days back.
HISTORY OF PAST ILLNESS
Patient is alcoholic since 15 years, last binge 10 days back.
N/k/c/o DM , BA, EPILEPSY.
PERSONAL HISTORY:
HABITS: alcoholic,he used to smoke few months back .
He follows a mixed diet.
Appetite -Normal,
Bladder movements- normal
Bowel movements- Normal .
Sleep- Adequate.
FAMILY HISTORY: No significant family history.
GENERAL EXAMINATION :
patient is conscious ,coherrent, co operative and well oriented to surroundings. He is moderately built and nourished.
